Key working is an approach involving the following areas of support
- Emotional and practical support including helping with difficult decisions
- Coordination including being your first point of contact
- Getting to know your child and family with a view to identifying needs and strengths in order to create a single plan agreed by all concerned
- Providing accurate information at the right time
Key working aims to ensure that the team around the child is working together effectively and to the same plan. Key working is prioritised when children are involved with a very large number of different professionals and organisations in order to help make sure that care plans are well coordinated. You may have one key worker or more than one, a multi-agency meeting and a plan which will work towards achieving the best outcomes for your child in partnership with you.
Who will have access to key working
- Families living in Sutton
- The child/children must be 0 – 8 years have complex needs arising from disability and/or complex health needs
- The child will require input from 3 or more specialist services over time
- Families who would most benefit from coordination of services
