Make a professional referral to adult social care

Report a safeguarding concern, or tell us about an adult who needs care or support.

You can make a professional referral if you're someone with a duty of care, such as a doctor, nurse, social worker, housing officer or police officer.

What you’ll need to tell us

We need enough information about someone to be able to identify and contact them.

We can only accept a referral if you tell us:

  • their name and date of birth
  • their phone number or email
  • their GP details
  • any health conditions
  • what they need help with

You don't need the person's permission to make a referral, but speak to them first if you can. We may not be able to help someone who doesn’t want it.

If you're reporting a safeguarding concern

If you're telling us about an adult at risk, we don't expect you to know all their personal details. Just give as much information as you can.

Occupational therapy assessment

This is for adults who might benefit from equipment or home adaptations because they struggle with daily tasks in the home. This could be because they:

  • are physically disabled
  • are recovering from an illness or operation
  • have learning disabilities
  • have mental health problems
  • are getting older

Also called a Care Act assessment. This is for adults who need help to:

  • wash or dress
  • access or use the toilet
  • stay safe around the home
  • access the community, leisure or work
